Some of the information and application forms below are the 2007 ones. The 2008 forms will be posted when they are received from AAHPERD. GAHPERD usually receives them by June 1. Applicants/nominees may wish to review these forms to become familiar with the criteria and process. However, all materials must be submitted on 2008 forms. Winners at the state level qualify for District and/or National Awards.
